徐根初，中將。上海人。1962年6月參加中國人民解放軍。大學本科文化。
歷任北京軍區后勤部計劃科長，后勤部參謀長。總后勤部副參謀長。1993年7月任后勤指揮學院院長，教授，博士生導師。1999年11月任軍事科學院副院長。
1988年被授予大校軍銜，1992年由大校晉升為少將軍銜，1999年晉升為中將軍銜。
