依據《中華人民共和國行政許可法》《衛生行政許可管理辦法》《醫療機構管理條例》等規定，下列醫療機構主動申請注銷《醫療機構執業許可證》，我委依法予以注銷。
以上醫療機構，我委已經依法注銷，任何單位和個人不得再以被注銷醫療機構名義（登記號識別）開展診療活動，違者將依法被追究法律責任。
